This Notice explains how Wurth Chiropractic Center and related entities (“WCC“ “we,” or “us”) collects and processes your Personal Data. This Notice is applicable to all Site(s) visitors, registered users, and all other users of our Site(s).

WHO WE ARE
For the purposes of the General Data Protection Regulation 2016/679 (the “GDPR”), the Data Controller is Wurth Chiropractic Center, a sole proprietorship with two locations: one is in the State of Illinois, USA, with an address at 201 W. Springfield, Champaign, Illinois 61820. The second is located n the State of Iowa, USA, with an address at 416 Main Street, Marcus, Iowa, 51035.
Dr. David C. Wurth is responsible for overseeing questions in relation to this Notice for the purposes of the GDPR.

HOW WE COLLECT PERSONAL DATA
Personal Data that you give us
We may collect and process the following Personal Data:
- Contact information, which you provide when corresponding with us by phone, e-mail or otherwise. This includes information you provide when you participate in discussion boards or other social media functions on our Site(s) and when you report a problem with our Site. - Patient health or treatment information will not be collected or transmitted through this website.

Where we store your personal data
The Personal Data that you provide to us is generally stored on servers located in the United States. If you are located in another jurisdiction, you should be aware that once your Personal Data is submitted through our Site, it will be transferred to our servers in the United States and that the United States currently does not have uniform data protection laws in place.
CHILDREN’S PRIVACY
Our Sites are not directed to children under the age of 13, if you are not 13 years or older, do not use our Sites. We do not knowingly collect online Personal Data from children under the age of 13. If we learn that Personal Data of persons less than 13 years-of-age has been collected through our Sites, we will take the appropriate steps to delete this information.
